1) Depends on the game. Some games are easier to modify than other, and there are communities out there. And some games are so heavily encrypted and compressed that it's nearly impossible.
2) In my opinion, yes. Moding is more of something that was sort of intended by the developer, and involves modifying files and the game complies with the changes. Hacking is more of modifying memory and code during and before run-time.

When people hack into a game what do they mean?
Answer
That depends on the context. It could mean someone who hacks the game itself or someone who is hacking in a game.
Hacking a game generally refers to hacking the program itself. For instance, figuring out a way to get around needing a disc to play the game or creating a serial number, or key, generator. People do this so that you can download a pirated copy and play it. There are problems with hacked games like this. Most games that offer an online version will not allow you to play with a hacked copy. Also, many hacks are buggy, don't have all the features available in the retail version, and tend to crash a lot. And yes, I know this from experience ;)
The other context is hacking IN an online game. This is when players use unauthorized software modifications (mods) or scripts to cheat. This is most often done in First Person Shooter games like Quake, Doom, Call of Duty, etc. Some examples of online hacks are aimbots (makes your shots incredibly accurate), wall hacks (see through or walk through walls), and map hacks (see where other players are on the game map). There are downsides here, as well.
A lot of online games use anti-cheating software, like Punk Buster, to detect and remove cheaters. Also, most server administrators will kick and ban cheaters. In fact, sometimes playing too well will get you accused of being a cheater and you'll get kicked.
