I'm writing a story, sort of a romance fanfic and I need a name for a sweet boy from district 4 whos in love with a rebellious girl named Maxima or Max, then I need a name for a sort of father figure to Max who works at the docks and helps her out well anyway help if you can

Where do authors come up with names for locations, characters, etc.?

I'm just curious. Do they think about random letter combinations that sound okay? Take Lord of the Rings for example: How did Tolkien come up with Sauron, Mordor, Frodo, etc.? Do they use random name generators?

Some name generators do help, seventhsanctum is one, and useful. Particular games might come in handy to create original ye olde medieval names like Medieval 2 Total War - can use the total war series for Roman, Celtic, Greek and Japanese names too.
But for a different take on names you could take real world names and split divide them and join them with others, that sound/fit to make a name/place/region that *sounds* exactly how you wanted it. For instance if you liked Latin or Spanish sounding names take that unique sound they have and if you wanted a Japanese sound take some recurring sounds there and gel the two together to make a sound that fits the characteristics (of Latin/Spanish and Japanese) together.
Edit: To answer your question on Tolkien and how he made up the names in his series, I know Gandalf for one was originally in the poem 'Beowulf' or that Finnish epic poem Kalevala (he may have taken the concept of Edoras there too) but Tolkien was a linguist so words like Mordor actually meant something in the Elvish language and things like that. Look at the Rohan culture, Eor is the Norse word for 'horse' you see a lot of that sound in the Rohan culture and names 'Eowyn' means 'Horse Joy' or someone who loves horses - finds love from horses, Eomer means man of horse and things like that. But, that doesn't mean you need to be a linguist to make mint sounding names.
